When you first visit one of our offices in Las Cruces or Roswell, NM, you will typically undergo a thorough oral examination. We will do an extensive review of your health history and complete any necessary X-rays or 3D scans to assist in an accurate diagnosis of your condition. These scans will also help us develop the most detailed and accurate treatment plan possible.
We have provided a list of materials below that you need to bring to your initial consultation. Ensuring you have these items helps your appointment run smoothly and helps us stay on time:
- A list of the medications you are currently taking
- Your medical history
- Your medical and/or dental insurance information (if applicable)
- Your referral form and any X-rays you were given
It’s possible that your dentist or doctor already sent over your X-rays when they filled out your referral form. If you’re not sure, or if you’re wondering if you need to bring in a referral form, please contact our office before your appointment.
Important Note: Patients under the age of 18 must have a parent or guardian accompany them to every appointment at our office, and they must stay for the entire duration of the visit.
